Vendor Questionnaires
These questionnaires should be completed by vendors for each project.
This form () must be completed by all INDIVIDUALS performing services.
Please note: after the form is completed by the vendor/consultant it will be forwarded to the 麻豆原创 contact, who will also need to sign the form.
ICQ form workflow
The form link is forwarded to the Vendor by the 麻豆原创 Department Contact
麻豆原创 Vendor completes the form online
麻豆原创 Vendor is asked to enter 麻豆原创 Department Contact information and verify email
The form is automatically forwarded to 麻豆原创 Department Contact
麻豆原创 Department Contract receives/reviews the form and signs to acknowledge the receipt
Once this is done, 麻豆原创 Vendor, 麻豆原创 Contract, OGC and Purchasing receive a copy of the signed ICQ form
ICQ form needs to be attached to all vendor's requisitions in Unimarket by 麻豆原创 Department Contract
Also, please note that the vendor needs to attach the most recent (not more than 1 year old) certificate of Harassment Prevention Training
The ICQ form needs to be submitted once a year. All vendors providing services to Teachers College need to submit it.

Vendor Addenda
These addenda should be signed by both parties and need to be completed in addition to the contract provided by the outside entity.
Complete this addendum in addition to the hotel鈥檚 own contract. The 麻豆原创 Hotel Addendum and hotel contract must be signed by both parties.
